All application materials that include fee payment must be mailed to DPI's Milwaukee Address below.

Mail (regular 1st class US mail only) application, all required documentation, and fee payment together to:

All mail that does not require a payment, such as correspondence or documentation mailed separate from the original application, should be sent to DPI's Madison Address.
